This is a document, written for the Network, by Pilgrims Hospice, which advises best practice for end of life care for heart failure patients. The document was written with the assistance of the community heart failure team and should prove very helpful in looking at the requirements of what is needed at end of life and how it can be provided. It also highlights the importance of multidisciplinary working.
